Green WB shield center labels w/ correct release title and "After Forever (Including The Elegy)" as A2, incorrectly credited to "Ioomi-Ward-Osbourne-Butler". Song durations are shown as they appear on center labels, but are obviously incorrect. 

Note that this release is similar to r4272661 (here), except that version's center labels incorrectly show "Masters Of Reality" as the title.

Catalog number "BS 2562" appears on back cover and center labels. "2562" appears on front cover only.
